Mental health screening tools are evidence-based questionnaires designed to help identify symptoms, support diagnostic clarity, measure symptom severity, and guide treatment planning. They are not diagnostic by themselves but provide valuable clinical information when combined with a comprehensive evaluation by a qualified mental health professional.

At Amazing Grace Center, completed screeners are automatically linked to the patient’s medical record, allowing clinicians to review results, monitor progress over time, and personalize treatment recommendations. Screeners may be completed during an appointment or securely sent to patients to complete before their visit, helping make appointments more focused and efficient.
