1. Discuss Pros and Cons With Your Patient

   Review our services, and insurances accepted with your patient to ensure we are have the right services for their care plan.

2. Call to provide referral Information

   • Patient demographics

   • Medical and psychiatric history

   • Current medications

   • Specific reasons for referral

3. Schedule the Appointment

   Direct patients to CALL 740-876-8449 • TEXT 740-414-0111 to schedule their initial evaluation for addiction treatment with integrative behavioral health services. Patients are also welcome to Walk-In to Amazing Grace Center at 802 Clare Ave. Portsmouth, OH 45662.
